School Immunization Requirements

Indiana law requires mandatory immunization against the following:

• diphtheria
• whooping cough
• tetanus
• polio
• rubella
• mumps
• hepatitis B

Parents must provide the school with complete immunization records prior to the beginning of the school year.  If your child has not been immunized due to medical or religious objections, written documentation is required.

Medication Policy

Parents are encouraged to schedule medication and/or medical treatments during non-school hours.  The term "medication" is not limited to prescription medicine, but includes over-the-counter medication, such as Tylenol or cough syrup.  Medications and/or medical treatments will be given only when there is a written physician order and parental consent.

All medication needs to be in the original container and labeled with the child's name.  The parent will assume responsibility for safe delivery to school.  Medication is to be kept in the clinic at all times, not on the child.  Exceptions include emergency medications for allergic reactions or for conditions that require immediate emergency treatment (i.e. asthma inhaler).

Senate Bill 376 allows students with emergency medication to carry and self administer medication provided they have written parental and physician approval.

Authorization for Medication and/or Treatment Forms can be obtained from the school nurse or front office.  Please call the school nurse if you have any questions concerning this policy.


Illness/Injury Recommendations

Students who are ill or seriously injured prior to the school day should be kept at home until improved (fever free for 24 hours) or released by a physician.  Please do not send ill or injured students to school to be diagnosed by the school nurse.

During the school day, if a student becomes too ill to remain in school or is seriously injured, reasonable effort will be made to contact the parents.  Parents are responsible for both transportation and follow-up care.  If a sudden, possibly life-threatening condition should occur, ambulance transportation to a hospital will be arranged.

School Health Screenings

Several health screenings are performed throughout the school year.  Vision screening tests are given to students in kindergarten, first, third, fifth, and eighth grades.  Dental exams are offered by volunteer dentists for students in kindergarten, second, and fifth grades.  Hearing tests are given to students under the direction of the speech and hearing therapist in kindergarten, first, fourth, seventh, and tenth grades.

You will be notified if your child fails any of these tests.  Please keep in mind that these are screening exams; they should not take the place of routine visits to your dentist, optometrist, or medical doctor.
